Patient Profile Tab

Introduction

The Patient Profile Tab provides a comprehensive overview of all patient information in one place. It displays demographics, clinical summary, current status, responsible staff, team assignments, and handover information. This is the central hub for viewing and managing patient details.

Before You Start

Permissions required:

  • To view the profile: You must have access to at least one team the patient is a member of
  • To edit demographics: Any user with access to the patient can edit basic information
  • To edit handover information: You must be a member of the specific team whose handover categories you want to update
  • To edit clinical summary, status, risk flag, location: Any user with access to the patient
How to Access the Patient Profile
  1. From the sidebar, click My Patients
  2. In the patient table, click on any patient row
  3. The Patient Profile Tab opens automatically (it's the default view)

What You'll See on the Profile Tab
Page Header

At the top of the page, you'll see:

  • Patient name and date of birth in the format: FirstName LASTNAME DD-MMM-YYYY
  • Status badge (coloured tag showing current status like "Stable" or "Critical")
  • "My Patient" badge (green tag, only shown if you're responsible for this patient)
  • Location badge (if a location has been assigned)
  • Print button (printer icon on the right)
  • Actions menu (three-dot kebab menu in the top-right corner)
Quick Action Buttons

If you share a team with the patient, you'll see two buttons below the tabs:

  • Create action — Opens a dialogue to create a new action for this patient
  • New note — Opens a dialogue to add a clinical note
Patient Thumbnail

On the left side of the demographics section, you'll see a gender-based avatar image (male, female, or other).

Demographics Section

The following information is displayed in a responsive grid:

  • First name
  • Last name
  • Age — Shown as: age in years (DD-MMM-YYYY format), or "-" if no date of birth
  • Gender — Male, Female, or Other
  • Nat.ID — National ID or NHS number
  • Medical Record # — Hospital number
  • Email
  • Cell / Mobile number
Editable Profile Fields

These fields have a pencil icon indicating you can click to edit them:

Risk flag

  • Shows risk level and reason (e.g., "High - Fall risk")
  • Click to open the Update Risk Flag dialogue

Current Location

  • Shows location as a coloured tag
  • Click to open the Move Patient dialogue

Open Actions

  • Shows count of open actions as a badge
  • Click to switch to the Actions tab

Status

  • Shows coloured status badge with status name
  • Displays time in status, last updated date/time, and who updated it
  • Click to open the Update Status dialogue
  • Note: Cannot edit if patient status is INACTIVE (pencil icon won't appear)
Read-Only Information Fields

Responsible users

  • Shows list of staff members responsible for this patient
  • Each name is clickable (links to their user profile)
  • Shows job title and speciality below each name
  • Displays "Nobody" if no one is responsible

Responsible teams

  • Shows list of teams the patient belongs to
  • Each team name is clickable (links to team view)
  • Displays "None" if patient not in any teams
Organisation-Specific Fields

Your organisation may have configured custom patient fields. Each custom field shows:

  • Field name (configured by your organisation)
  • Current value (or "-" if not set)
  • Last updated timestamp
  • Pencil icon to edit

Click any custom field to update it. The edit dialogue will show:

  • Current value (read-only)
  • Input for new value (type varies: text, number, date/time, or yes/no)
  • Optional toggle: "Is this old/historic data?"
  • If historic data: fields for applicable date and time
Clinical Summary Section

A large text area showing the patient's clinical summary or diagnosis.

  • Shows "Patient has no clinical summary" if none has been entered
  • Click anywhere in this section to update the clinical summary
  • Has a pencil icon in the top-right
Handover Information Section

At the bottom of the Profile tab, you'll see handover information organised by team. Each team the patient belongs to has its own section labelled "TeamName Handover Information".

For each handover category:

  • Category name (configured by team owners)
  • Category value (text or structured form data)
  • Last updated date/time and user name

If a category has multiple entries (amendable categories), you can navigate through them using carousel arrows.

To edit handover information:

  • Click on any handover field
  • The Edit Team Info dialogue opens
  • Important: You can only edit handover information for teams you're a member of. If you try to edit a team you don't belong to, you'll see the message: "You must be a member of the same team to edit this information"
Editing Patient Demographics
  1. Click the kebab menu (three dots) in the top-right corner
  2. Select Edit Demographics
  3. The Edit Patient dialogue opens, showing the current patient information
  4. Modify any fields as needed:
    • Required fields: First Name, Last Name (marked with *)
    • Optional fields: Date of birth, Gender, Nat.ID, Medical Record Number, Email, Cell / Mobile number
  5. For Gender, select from the dropdown: Male, Female, or Other
  6. For Date of birth, use the date picker. You must select a complete date (day, month, year) or leave it blank
  7. Click Submit to save your changes

Validation:

  • First Name and Last Name are required
  • If you partially enter a date of birth without completing it, you'll see a validation error
  • Email must be in valid format if provided

The dialogue closes and the profile refreshes with the updated information.

Editing Other Profile Fields
Updating Risk Flag
  1. On the Profile tab, find the Risk flag field (has a pencil icon)
  2. Click anywhere on the field
  3. The Update Risk Flag dialogue opens
  4. Select the risk level and enter a reason
  5. Click Submit to save
Changing Patient Status
  1. Find the Status field (has a pencil icon)
  2. Click anywhere on the field
  3. The Update Status dialogue opens
  4. Select a new status from available options
  5. Click Submit to save

The status badge updates immediately, showing:

  • New status name and colour
  • Time in status (resets to current time)
  • Your name as the person who updated it

Note: You cannot edit status if the patient is INACTIVE (the pencil icon won't appear).

Changing Patient Location
  1. Find the Current Location field (has a pencil icon)
  2. Click anywhere on the field
  3. The Move Patient dialogue opens
  4. Enter or select the new location
  5. Click Submit to save
Updating Clinical Summary
  1. Scroll to the Clinical summary section
  2. Click anywhere in the section (has a pencil icon)
  3. The Update Diagnosis dialogue opens
  4. Enter or update the clinical summary text
  5. Click Submit to save

The summary updates and shows the timestamp of the last update.

Editing Organisation Fields
  1. Find the organisation-specific field section (below demographics)
  2. Click on any custom field (each has a pencil icon)
  3. The Edit Organisation Field dialogue opens
  4. Enter the new value (type depends on field configuration)
  5. If this is historic data, toggle "Is this old/historic data?" to Yes
  6. If historic: Enter the applicable date and time
  7. Click Submit to save

The field updates showing the new value and timestamp.

Using the Actions Menu

Click the kebab menu (three dots) in the top-right corner to access additional actions:

Always available:

  • Edit Demographics — Edit patient's basic information
  • New note — Create a new clinical note

If you're responsible for the patient:

If you share a team with the patient (but aren't responsible):

If patient has been discharged from all teams:

If you created the most recent note less than 1 hour ago:

Tips
  • Quick navigation: Click the "Open Actions" field to jump directly to the Actions tab
  • User and team links: Click on responsible user names or team names to view their profiles or team pages
  • Empty fields: A dash "-" indicates no value has been set for that field
  • Print function: Use the printer icon to print the entire patient profile
  • Time-sensitive edits: You can only edit your latest clinical note if it's less than 1 hour old
  • Handover permissions: If you can't edit handover information, check that you're a member of that team
