Working with Actions and Notes

Introduction

CAREFUL keeps actions and clinical notes as separate features. You cannot create an action directly from a clinical note. However, you can create actions for patients, add progress updates to existing actions, and the system automatically creates clinical notes when you close actions.

Before You Start
  • You must have a team in common with the patient to create actions or clinical notes
  • You must be logged in as an organisational user (Administrator, Team Owner, or Clinical User)
Understanding Actions and Notes

Actions are tasks assigned to team members with optional due dates. They track work that needs to be completed for a patient.

Clinical Notes are free-text summaries documenting patient care. They appear in the patient's Notes tab.

The Connection: When you close (complete) an action, CAREFUL automatically creates a clinical note titled "Action Closed - [action name]" with a link back to the action.

Creating a New Action
  1. From the sidebar, select My Patients
  2. Click on the patient's row to open their patient view
  3. Click the Create action button (above the tabs)
  4. In the dialogue box:
    • Enter the action Title (required)
    • Select an Assignee by typing their name (required)
    • Optionally set a Due date and Due time
    • Optionally add a Detailed Description
    • Optionally attach files using the Attachments uploader
  5. Tick Create another after submit if you want to create multiple actions in succession
  6. Click Submit

The dialogue box closes and the action appears in the patient's Actions tab.

Adding Progress Notes to an Existing Action
  1. From the sidebar, select My Patients
  2. Click on the patient's row to open their patient view
  3. Click the Actions tab
  4. Find the action in the table and click its kebab menu (⋮) on the right
  5. Select Update Progress
  6. The "Update progress on action" dialogue box opens, showing any previous updates
  7. Enter your update in the New update field (required)
  8. Optionally attach files using the Attachments uploader
  9. Click Submit

The dialogue box closes and your progress note is added to the action's history.

Creating a Clinical Note
  1. From the sidebar, select My Patients
  2. Click on the patient's row to open their patient view
  3. Click the New note button (above the tabs)
  4. In the "Update notes" dialogue box:
    • Set the Applicable Date (defaults to today)
    • Optionally enter a Title
    • Enter your clinical summary in the New note field
    • Optionally attach files using the Attachments uploader
  5. Click Submit

The dialogue box closes and the note appears in the patient's Notes tab.

How Actions Link to Notes Automatically

When you complete an action:

  1. Navigate to the patient's Actions tab
  2. Click the kebab menu (⋮) next to the action
  3. Select Complete Action
  4. Enter the result or outcome
  5. Click Submit

CAREFUL automatically creates a clinical note titled "Action Closed - [action name]" in the patient's Notes tab. This note includes a Show more.. link that navigates back to the closed action.

Tips
  • Actions and notes are independent features — you cannot create one from the other
  • If you need both an action and a note for the same patient, create them separately using the Create action and New note buttons
  • Progress notes added to actions (via Update Progress) are different from clinical notes and only appear within the action's history
  • To see which actions are linked to closed clinical notes, view the note in the Notes tab and look for the "Associated Actions" section
