View Clinical Summary History

Introduction

The Notes tab on a patient's profile displays the complete clinical summary history for that patient. This allows you to review all past clinical notes, assessments, and documentation created by your team, providing a comprehensive audit trail of clinical care.

Before You Start
  • You must have access to the patient (be a member of at least one team the patient is assigned to)
  • No special permissions are required to view clinical summaries
Steps
Accessing Clinical Summary History
  1. Navigate to the patient's profile by clicking on their name from any patient list (My Patients, a Team patient list, or My Organization patients tab)
  2. Click the Notes tab at the top of the patient profile
  3. The clinical summary history table loads, displaying all notes in reverse chronological order (most recent first)
Understanding the Clinical Summary Table

The table displays the following information for each clinical summary:

  • Date Created – The date and time when the summary was created
  • Applicable Date – The date the clinical information relates to (may differ from creation date)
  • Created by – The name of the user who created the summary
  • Attachments – A badge showing the number of attachments, if any
  • Title – The summary title
  • Content – A preview of the summary content, or a Show more.. link for detailed summaries
Viewing Detailed Summary Information
  1. For JSON-format clinical summaries (typically structured assessments), click the Show more.. link in the Content column
  2. A dialog box opens displaying the full summary with two sections:
    • Form Data – A table showing all form fields with their descriptions and values (Yes/No for checkboxes, formatted dates/times for temporal fields)
    • Associated Actions – A list of any actions created as part of this summary, showing the action name and due date
  3. Click the close button (×) or click outside the dialog to return to the Notes tab
Viewing Action-Related Summaries
  1. For summaries marked as "action_closed" format (created when an action is completed), click the Show more.. link
  2. You will be taken directly to the action's detail page where you can see the full context
Viewing Attachments
  1. If a summary has attachments (indicated by a numbered badge in the Attachments column), click the action menu (⋮) on that row
  2. Select View Attachments from the menu
  3. The attachments carousel opens, allowing you to view all attached images or documents
Editing Recent Notes
  1. Only the most recent clinical summary can be edited, and only if you created it less than 1 hour ago
  2. If eligible, click the action menu (⋮) on the latest summary row
  3. Select Edit note from the menu
  4. The amendment dialog opens where you can update the summary details

Note: Summaries created from completed actions (action_closed format) cannot be edited.

Tips
  • If the table is empty, the message "This patient has no notes to show" displays – this indicates no clinical summaries have been created yet
  • The clinical summary history automatically refreshes when a new note is created or an existing note is amended
  • DateTime fields display in your local timezone format
  • Boolean fields (Yes/No questions) display as "YES" or "NO" in the detailed view
  • Use the Applicable Date column to find summaries related to a specific date of care, which may differ from when the note was actually documented
