Introduction
The Notes tab displays a complete history of clinical notes recorded for a patient. You can view all notes, create new entries with attachments, and edit recent notes you've created.
Before You Start
- You must have access to the patient through a shared team to view the Notes tab
- To create new notes, you must share a team with the patient
- You can only edit notes you created yourself, and only within 1 hour of creation
Steps
Viewing Patient Notes
- From the sidebar, select My Patients or navigate to a specific team
- Click on a patient row to open their patient record
- Click the Notes tab (the third tab after Profile and Actions)
- The notes table displays with the following columns:
- Date Created – when the note was recorded
- Applicable Date – the date the note applies to clinically
- Created by – the staff member who wrote the note
- Attachments – number of attached files (if any)
- Title – the note's title
- Content – the note text or a "Show more.." link for detailed entries
Viewing Note Details
- For text notes, the content displays directly in the table
- For structured (JSON) notes, click the Show more.. link to open a detailed view showing form data and associated actions
- For notes created when an action was closed, click Show more.. to navigate to that action's details
Viewing Attachments
- Locate a note with attachments (indicated by a number badge in the Attachments column)
- Click the kebab menu (⋮) at the end of the note row
- Select View Attachments
- A carousel viewer opens displaying all attached images
- Navigate between images using the arrow controls
- Click the close button to dismiss the viewer
Creating a New Note
- From any tab on the patient record, click the New note button at the top of the screen (or select New note from the patient kebab menu in the top-right corner)
- The "Update notes" dialogue box appears
- Complete the form fields:
- Applicable Date (required) – select the date this note applies to using the date picker. You can select dates up to 25 years in the past
- Title (optional) – enter a brief title for the note
- New note – type your clinical note in the text area
- Attachments (optional) – click the file uploader to attach images or documents. You can upload multiple files
- Click Submit
- The dialogue closes and your new note appears at the top of the notes table
Editing the Latest Note
You can amend the most recent note if you created it and it's less than 1 hour old:
- Click the patient kebab menu (⋮) in the top-right corner and select Edit Latest Note
(Alternatively, click the kebab menu on the latest note row and select Edit note) - The "Amend latest notes" dialogue appears with your note text pre-populated
- Edit the text in the New update field
- Click Submit
- The dialogue closes and the updated note content displays in the table
Tips
- Notes appear in chronological order with the most recent at the top
- If you see "This patient has no notes to show", no clinical notes have been recorded yet
- You cannot edit notes created by other staff members
- After 1 hour, notes become read-only and cannot be amended
- The Submit button is disabled while files are uploading – wait for uploads to complete before submitting
- On mobile devices, the Attachments column is hidden but you can still access attachments via the kebab menu
