How to Readmit a Patient

Introduction

When a patient who was previously discharged needs to return to your care, you can readmit them to a team. This restores their active status and assigns them to the appropriate clinical team for ongoing management.

Before You Start
  • You must be a member of at least one team to readmit a patient
  • The patient must be fully discharged (not currently assigned to any teams)
  • You need to know which team the patient should be readmitted to
Steps

There are two ways to readmit a patient, depending on where you start:

Method 1: From the Patient Profile

Use this method when you're already viewing a discharged patient's profile.

  1. Navigate to the patient's profile page (you can find them via search or from your patient lists)
  2. Click the kebab menu (⋮) in the top navigation bar
  3. Select Readmit from the dropdown menu (this option only appears for fully discharged patients)
  4. The "Readmit patient" dialogue box appears
  5. If you belong to multiple teams, select the team from the Team dropdown. If you only belong to one team, it will be pre-selected
  6. Optionally, select a patient status from the dropdown (e.g., "Admitted", "Under review"). You can also select "No status"
  7. Click the Submit button

The dialogue closes, and the patient is readmitted to the selected team. You'll be taken to the patient's profile page, where you can now see their active team assignment.

Method 2: From the Team View

Use this method when you're adding an existing patient to your team.

  1. From the sidebar, click on the team name under Teams
  2. Click the kebab menu (⋮) in the top navigation bar
  3. Select Add patient from the dropdown menu
  4. The "Find a Patient" dialogue box appears
  5. Enter the patient's name, NHS number, or hospital number in the search field
  6. Select the patient from the search results
  7. The dialogue transitions to "Add a Patient" with the patient's details displayed
  8. The Team field is pre-populated with your current team
  9. If the patient is currently inactive, you'll see their current clinical summary. You can add a new clinical summary if needed
  10. Optionally, select a Patient status from the dropdown
  11. Enter the referral source if required by your organisation
  12. Click the Submit button

The patient is readmitted to your team and you're taken to their profile page.

Tips
  • No teams warning: If you're not a member of any teams, you'll see a warning message: "You are not a member of any teams. You cannot readmit a patient unless you are a member of at least one team." You'll need to join a team before you can readmit patients
  • Patient already in team: If you try to readmit a patient who is already assigned to the selected team, you'll receive an error message. Check the patient's current team assignments on their profile
  • Taking responsibility: When you readmit a patient, you automatically take responsibility for them if you weren't already responsible
  • Status is optional: You don't have to assign a patient status when readmitting. You can leave it as "No status" and update it later from the patient's profile
