How to Handover an Individual Patient

Introduction

When you need to transfer responsibility for a patient to another team member—such as at the end of your shift or when going off duty—you can send them a handover request. The handover transfers both patient responsibility and all outstanding actions to the receiving clinician once they accept.

Before You Start
  • You must be the currently responsible user for the patient you want to handover
  • You cannot have an existing pending handover request for this patient
  • The person you're handing over to must be on duty and share a team with the patient
Steps
  1. From the sidebar, select My Patients to view your patient list, or navigate to a team and view the Patients tab

  2. Click on the patient's name to open their patient profile

  3. In the top-right corner of the patient view, click the kebab menu (three dots ⋮)

  4. Select Handover Patient from the dropdown menu

    Note: This option only appears if you are currently responsible for the patient

  5. A dialog box opens with the title "Request patient handover for [Patient Name]"

    The dialog explains: "You are about to handover [Patient Name]. This will send a request to the user you select below and if they accept, responsibility along with all outstanding actions will be assigned to them."

  6. In the search field (placeholder text: "Start typing a name to assignee to search..."), begin typing the name of the clinician you want to handover to

  7. As you type, autocomplete suggestions appear showing each user's name, job title, and specialty

    Note: Users who cannot accept the handover (e.g., "Not on duty" or "Not a member of the team") appear in the results but cannot be selected

  8. Click on the name of the clinician you want to handover to

  9. Once selected, the user's details appear below the search field, displaying their avatar, name, and role

  10. Review the details and click the Send handover request button

  11. The dialog closes automatically and your handover request is created

What Happens Next
  • The target clinician receives a notification: "has requested to handover one of their patients" (if they are on duty)
  • Your handover request appears in the Sent tab of the Handovers view (accessible from the sidebar)
  • The target clinician sees your request in the Received tab of their Handovers view
  • If they accept the handover: responsibility and all outstanding actions transfer to them immediately
  • If they reject the handover: the request is cancelled and you remain responsible for the patient
Tips
  • Already have a pending handover? If you see an error message saying "You cannot handover [Patient] because you have already issued a handover request for the patient to [User]", you must first cancel the existing handover from the Handovers view (Sent tab) before creating a new one

  • User appears greyed out or marked as invalid? This means they cannot currently accept handovers—they may be off duty or not a member of a team shared with this patient. Select a different clinician who is on duty

  • Handover all your patients at once? If you're going off shift and need to handover multiple patients, use the Handover all patients option from the kebab menu on the My Patients view, or Handover all my patients in this team from a team's kebab menu

  • Just want to step back from a patient? If you don't need a specific person to take over and just want to relinquish responsibility, use Relinquish responsibility instead. This allows any team member to take responsibility later
