CAREFUL complements your existing clinical systems. It does not replace your EPR — it fills the coordination gap that EPRs were not designed to cover.
Integration engine
CAREFUL uses Mirth Connect — a widely deployed, open-source healthcare integration platform with a proven NHS track record. Mirth supports HL7v2 ADT messaging, FHIR and direct API integration.
How the ADT feed works
1. Inbound. Your PAS sends HL7 ADT messages (admit, transfer, discharge, update) to Mirth Connect via secure VPN.
2. Routing. Mirth parses each message and applies configurable rules. Location codes map to CAREFUL teams — configured once during implementation, updatable by Trust IT without code changes.
3. Action. On admission or transfer, the patient appears in the correct CAREFUL team automatically. On discharge, they are removed. No manual list maintenance.
4. Queueing. RabbitMQ message buffering ensures no messages are lost during high-volume periods.
Manual patient lookup
For patients not admitted via ADT — outpatients, quality follow-ups, rare disease cohorts — CAREFUL provides direct PAS lookup. Clinicians search by NHS number, hospital number or name and add patients to any team.
What demographic data is used
CAREFUL maintains a minimum dataset: name, gender, date of birth, hospital number, NHS number and contact details. This data travels with referrals, appears on handover sheets and links to audit trails — ensuring positive patient identification across every interaction.
EPR integration
CAREFUL can send structured notes and actions to your EPR via FHIR messaging or PDF export. The EPR remains the record of truth. CAREFUL coordinates the work that generates those records.
For Altera Sunrise EPR specifically, CAREFUL has an established relationship with Altera's UK leadership team, including their CTO and CMO, who support CAREFUL integration to both PAS and Sunrise EPR.
Multi-organisation integration
The same integration pattern scales across organisations. Through a partnership with Leecare in Australia, CAREFUL integrates across 1,000+ care facilities using both ADT feeds and search — demonstrating reliable multi-site, multi-organisation integration at scale.
