Team Referrals Tab

Introduction

The Team Referrals tab allows you to view and manage patient referrals between teams. You can see all referrals your team has received from other teams, as well as referrals your team has sent out. This helps coordinate patient transfers and specialist consultations across your organization.

Before You Start

  • You must be a member of the team to view its Referrals tab
  • The tab displays a badge showing the total number of outstanding referrals (received + sent)
  • If you see "You do not have permission to view the referrals of this team," contact a Team Owner or Administrator

Viewing Team Referrals

  1. From the sidebar, click Teams to expand the accordion
  2. Select the team you want to view
  3. Click the Referrals tab in the tab bar

You'll see two tables:

Referrals received (top section) shows referrals sent TO your team from other teams. Each row displays:

  • Patient name (clickable link to their profile)
  • Source team (the team sending the referral)
  • Note (reason for referral)
  • Attachments (with count and view button if files are attached)
  • Action buttons: Accept (green with checkmark) and Reject (red with cross)

Referrals sent (bottom section) shows referrals your team has sent TO other teams. Each row displays:

  • Patient name (clickable link)
  • Target team (the receiving team)
  • Action button: Cancel (red with cross)

Viewing Referral Attachments

  1. Locate the referral in the Referrals received table
  2. If attachments are present, you'll see a count badge (e.g., "2")
  3. Click the view button
  4. An attachments carousel opens showing all files and images
  5. Navigate through attachments using the arrows
  6. Click the close button to dismiss the carousel

Accepting a Received Referral

  1. In the Referrals received table, locate the referral you want to accept
  2. Review the patient name, source team, note, and any attachments
  3. Click the Accept button (green with checkmark icon)
  4. The button shows a loading spinner while processing
  5. Once accepted, the referral disappears from the table and the patient is added to your team

If an error occurs, a dialog titled "Referral Error" will appear with details. The referral remains in the table so you can retry.

Rejecting a Received Referral

  1. In the Referrals received table, locate the referral you want to reject
  2. Click the Reject button (red with cross icon)
  3. The button shows a loading spinner while processing
  4. Once rejected, the referral disappears from the table
  5. The sending team is notified and the referral is removed from their sent list

Canceling a Sent Referral

  1. In the Referrals sent table, locate the referral you want to cancel
  2. Click the Cancel button (red with cross icon)
  3. The button shows a loading spinner while processing
  4. Once cancelled, the referral disappears from the table
  5. The receiving team no longer sees it in their received referrals

Initiating a New Referral

To send a referral from your team to another team:

  1. Navigate to the patient's detail page
  2. Click the kebab menu (⋮) in the patient header
  3. Select Refer Patient to Team
  4. In the dialog that appears, fill in:
    • From: Your source team (auto-filled if you're only in one team)
    • To: Select the target team from the dropdown
    • Reason for Referral: Enter clinical details (SBAR format suggested)
    • Attachments: Optionally upload supporting documents
    • Transfer Immediately?: Check this box to bypass the acceptance process and transfer the patient immediately (this will auto-accept the referral and discharge from your team)
  5. Click Refer
  6. If successful, the referral appears in your Referrals sent table and the target team's Referrals received table

Tips

  • Immediate transfers: Use the "Transfer Immediately?" checkbox when you've already coordinated with the receiving team and don't need their explicit acceptance
  • SBAR format: When entering referral notes, consider using SBAR (Situation, Background, Assessment, Recommendation) for clear clinical communication
  • Button states: Accept and Reject buttons are disabled while processing to prevent duplicate submissions
  • Real-time updates: After accepting, rejecting, or canceling a referral, the table automatically refreshes to show current status
  • Empty states: If you see "This team has no outstanding received referrals" or "This team has no outstanding sent referrals," your team has no pending referrals in that category
  • Audit trail: All referral actions (requested, accepted, rejected, cancelled) are logged in the audit trail for compliance
