How to Readmit a Patient

Introduction

When a patient who was previously discharged needs to return to your care, you can readmit them to a team. This restores their active patient record and assigns them back to a care team with an optional status.

Before You Start

  • You must be an organisation user (not a system user)
  • You must be a member of at least one team
  • The patient must be currently discharged (not a member of any team)

Steps

From a Patient Profile:

  1. Navigate to the discharged patient's profile page
  2. Click the Readmit button in the patient actions menu (shown with an enter icon)
  3. The Readmit patient dialog box appears
  4. Select a team:
    • If you belong to only one team, it will be automatically selected and displayed
    • If you belong to multiple teams, select the appropriate team from the Team dropdown
  5. (Optional) Select a Patient status from the dropdown:
    • Choose No status to readmit without a specific status
    • Or select a status like "Admitted", "Under review", etc.
  6. Click the Submit button
  7. The dialog closes and you're automatically navigated to the patient's profile page
  8. The patient now appears in your team's patient list

From Patient Search:

You can also readmit patients directly from the global search or team patient search. The workflow is the same - click Readmit on the search result, select team and status, then click Submit.

Tips

  • Team pre-selection: When readmitting from a team view, the team may be pre-selected automatically
  • No status option: It's perfectly fine to readmit without setting a status - you can update the status later from the patient profile
  • Multiple teams: If you're a member of several teams, make sure to select the correct team for the patient's care pathway
  • Audit trail: All readmissions are recorded in the patient's audit log with a "Readmitted" entry
  • Can't see the button?: If you don't see a Readmit button, check that:
    • The patient is actually discharged (not currently in any team)
    • You are a member of at least one team
    • You have organisation user permissions
