How to Add a New Action for a Patient

Introduction

Actions are tasks or to-dos that need to be completed for a patient's care. You can create actions for yourself or assign them to other team members, with optional due dates and attachments to track important care activities.

Before You Start

  • You must be a member of at least one team in common with the patient
  • You need to be viewing the patient's profile or have access to their chat messages
  • No special permissions beyond team membership are required

Steps

From the Patient Profile Page

  1. Navigate to the patient's profile page by selecting them from your patient list
  2. Click the Create action button located above the patient tabs, or click the dropdown menu next to the patient's name and select Create Action
  3. The "Create new action for patient [Name]" dialog appears with your name pre-filled as the assignee
  4. Enter a title for the action in the Title field (required)
  5. To assign the action to someone else, click the Assignee field and start typing their name. Select the user from the search results. Invalid users will appear grayed out with a reason
  6. (Optional) Expand the Optional Information section to add:
    • Due date: Select the year, month, and day from the dropdowns
    • Due time: Select the hour and minute (in 24-hour format, 5-minute increments)
    • Attachments: Click to upload files related to this action
    • Detailed Description: Add additional notes or context
  7. If you need to create multiple actions, check the Create another after submit checkbox
  8. Click the Submit button
  9. The dialog closes and the new action appears in the patient's action list. If you checked "Create another after submit", the form resets and stays open for you to create another action immediately

From Chat Messages

  1. In any chat room, locate a message that mentions both a patient and a user
  2. Click the + button that appears on the message
  3. The create action dialog opens with the mentioned user pre-selected as the assignee
  4. Follow steps 4-9 above to complete the action creation

Tips

  • The Title field is automatically focused when the dialog opens, so you can start typing immediately
  • By default, actions are assigned to you. You only need to change the assignee if someone else should handle the task
  • If you start entering a due date, you must complete all three fields (year, month, day) before submitting
  • The Submit button is disabled while files are uploading, so wait for uploads to complete
  • Use the "Create another after submit" feature when you need to create multiple actions for the same patient quickly—it keeps the assignee selected and opens a fresh form

Common Issues

  • Submit button is disabled: Make sure you've entered a title and selected a valid assignee
  • Validation error on due date: If you started entering a date, you must complete all three dropdowns (day, month, year)
  • Cannot select a user: Some users may appear grayed out with a reason in parentheses—they cannot be assigned actions
