How CAREFUL Works With Your EPR

What EPRs do well

EPRs store clinical data: notes, results, medications, diagnoses. They provide a longitudinal record. They support order entry, prescribing, and reporting. For documentation and record-keeping, the EPR is essential and irreplaceable.

The coordination gap

EPRs were designed to document, not to coordinate. They record that a referral was requested — but do not track whether it was accepted, actioned or completed. They capture a discharge plan — but do not show which team is responsible for each step, or whether the steps are done.

The coordination work — chasing referrals, tracking actions, handing over tasks, communicating between teams — happens outside the EPR. On paper. In WhatsApp. Over bleeps. In someone's memory.
