The patient profile screen can act as a clinical handover sheet. It can be populated with all the information needed for a thorough handover and can be shared across boundaries.
The patient profile includes the following information:
- Patient demographics - e.g. hospital number, DOB or contact details
- The responsible teams
- The patient's status in their clinical journey
- Risk flag
- Organisation-specific fields - e.g. expected discharge, frailty score and dementia
- Clinical summary
- Handover information for patient transfer across boundaries
All of these fields can be edited by clicking on the pencil sign beside them
