Bringing together multi-professional teams across boundaries using one secure platform
The secure clinical platform for multi-professional teams anywhere.
CAREFUL allows teams to coordinate clinical care — even if they are in different organisations.
Healthcare practitioners in hospitals, clinics and as community providers can make rapid decisions by sharing important information quickly and easily using either desktop or mobile.
CAREFUL can integrate into existing systems or be used stand alone. Users can self-customise and configure their teams, views and how they manage their patients.
CAREFUL helps keep track of who needs to do what next and which teams are responsible, together with all handovers and referrals. It replaces the inefficient tools with which doctors and nurses struggle to coordinate care, such as WhatsApp, emails, bleeps and spreadsheets.
It is a clinically-proven platform where everyone can collaborate and communicate seamlessly for better outcomes.
CAREFUL provides a shared view of cohorts of patients. This shows all the actions for every team, so everyone can see the same plan and knows who should be doing what well into the future.
This level of coordination reduces errors, creates better outcomes for practitioners and patients through continuity of care.
Helps hospitals reduce length of stay, minimise delays to discharge and maximise bed capacity.
Links primary care, hospital specialists and other healthcare providers. This creates continuity of care and helps adherence to treatment and follow up.
Supports specialist case management to ensure faster, safer and audited decisions are made wherever possible.
Lets doctors and nurses communicate easily between larger institutions, saving valuable time and reducing mis-steps during complex transfers.
Creates a shared care plan for GPs, pharmacists and community practitioners, reducing the risk of errors and avoiding hospital admissions.
Manages population health targets. This improves value-based care to help patients, providers and payers.
The CAREFUL team are here to help.
Have a question about CAREFUL? Reach out and we will respond as soon as we can.
