15 January 2055 – We are proud to announce that CAREFUL has been selected by City Cancer Challenge (C/Can) to enhance care coordination for cancer patients in their growing network of cities as part of their digital solution library.
Coordinating cancer care is a challenge. The complexity of care delivery for a cancer patient demands orchestrated efforts across multiple medical specialities, support services and healthcare facilities.
Cancer treatment involves intricate pathways that require precise coordination among oncologists, pathologists, radiologists, specialised nurses and nurse navigators, physical therapists, nutritionists, social workers, palliative care specialists, pharmacists, mental health professionals and other health practitioners. Where resources are constrained or access is limited, the right oversight, in a timely manner, by multidisciplinary teams is often difficult.
Each patient’s journey involves multiple touchpoints, transitions of care and critical decision points. A breast cancer patient, for example, may interact with up to 15 different healthcare professionals across various facilities during their treatment journey.

As part of C/Can’s digital health library toolkit, we are committed to jointly helping to improve patient navigation and address the complexities of fragmented healthcare systems. With shared management of lists of patients, their key clinical summaries, actions and next steps, everyone in the patient’s care teams can follow progress, refer, handover and manage current and future treatment. All using a clinically intuitive and easy to use mobile app.

We will also help build capacity for digital health adoption and measure and share impact data.
